Passport Name Change
In order to change the name on your passport you must have an original court ordered name change, original marriage certificate or divorce decree.
You must renew your passport in order to change your name. If your passport was issued less than a year from the date of issue and you need your name changed or corrected in your passport, you will only need to pay the expedite fee to the US State Department to have it corrected.
How to Apply
- Do not sign the application until instructed to do so.
- You must provide a Social Security number, if you have been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f)
- You must submit your current undamaged passport.
- The expired passport will be returned to you along with the new passport.
You must provide the original or certified copy of the name change document from the court.
- If you have a copy of your flight itinerary or ticket, please include it.
- If you do not have a travel itinerary or a ticket, it’s okay.
- Passport photo services available at our offices.
- Must be 2″ x 2″ on a white background.
- Photo must not be older than one month.

My passport was issued less than a year ago – do I still need to pay full fees?
No. If your passport was issued less than one year ago and you need the name changed or corrected, you only need to pay the expedite fee to the US Department of State – the full government renewal fee does not apply. The government fee in this case is $60.00 instead of $213.36.
Can I apply for a passport name change by mail through Sameday?
